Although hair transplant has become a common and preferred procedure, patients still have some questions in their mind considering the operation. One of the first questions is about the location of the first transplant.

Since the factors affecting hair loss and changes in the physiological structure of the person are different from each other, it is impossible to give clear information about the duration and the type of hair loss. However, frequent tests can determine the average course.

There are two main areas in the scalp when we consider a hair transplant.

Crown (Vertex)

The vertex, a Latin word, comes from the word vertic, which means “peak.” In some men, the hair loss in the vertex is faster than the frontal region. Therefore, a person may first notice hair loss in the crown region.

Since the crown region is out of eye level, patients do not consider this area as important as the forehead hairline. Besides, hair transplant performance in the crown region is less than the anterior hairline. The reasons for this performance obtaining a natural result in the crown region are

• less blood circulation in that region,
• inability to reach a sufficient number of grafts because of the large area, and 
• the difficulty of obtaining proper shape.

Hair transplanted to the crown region reaches its natural form after approximately twelve months. In this region, the scalp is a little harder and thicker. Therefore, the scalp cannot feed the roots successfully, and the hair follicles grow slowly. It is a little more difficult to close this place in a single session and achieve satisfactory results for the same reasons.

Hairline

Hair transplant is an important operation that requires knowledge and experience. The most critical factor that determines the success and naturalness of hair transplant is front hairline design.

The following are some points to consider when determining the hairline. 

• Gender
• Head shape
• Age
• Facial structure
• Number of grafts
• The desired hairstyle
• Hair loss density 

From a medical point of view, the front hairline is considered the most important region when it comes to natural appearance in hair transplant operations. On the other hand, the crown (vertex) region is the most challenging area for hair restoration in order to achieve a natural appearance. The hair follicles in the vertex, which is a region with its own arithmetic within a large area, has its own dynamics.

The hairline is the part where the first eye contact happens. Therefore, hair transplant, which may require two sessions, starts from the forehead line and continues gradually towards the crown. Usually, the first session forms 70 per cent of the whole operation. 

As we give a denser look in the first session, the patient sometimes says, “Let the back of my head be a little bald. What seemed important to me was the front, and it looks impressive.” Therefore, we recommend starting from the front hairline.
